Financial Assistance

Community Health Services of Lamoille Valley (CHSLV) is a Federally Qualified Healthcare Center and non-profit corporation with a goal to provide quality health care services, regardless of a patient’s ability to pay. Good health is the most important thing in life; paying for it shouldn’t make you sick. Our financial assistance team works with patients to help reduce concerns that their medical bills will lead to financial hardship. CHSLV offers a financial assistance program and you may qualify for significant savings on your CHSLV medical bills. How do you know if you qualify for financial assistance?

Step 1. Use this calculator to determine qualification and discount.

  • Type in number of family members
  • Type in your annual income. Use a period (not a comma), to separate numbers
  • The applicable discount will appear in the “Discount Percent” box*

*There are nominal charges based on percentage discount:
100% discount there is a $5.00 nominal charge
75% discount there is a $10.00 nominal charge
50% discount there is a $15.00 nominal charge
25% discount there is a $20.00 nominal charge

Step 2. If you qualify for financial assistance, download and complete the
Financial Assistance Form.
It’s all confidential and you will be treated with the high level of respect you’ve come to expect at CHSLV. Return the completed form, along with your proof of income to your Physicians Office or mail to: CHSLV, P.O. Box 749, Morrisville, VT 05661

Community Health Services of Lamoille Valley (CHSLV) is funded in part through a grant from the U.S. Health and Human Services Administration and generous community support. CHSLV is a Health Center Program grantee under 42 U.S.C. 254b and a deemed Public Health Service employee under 42 U.S.C. 233

This health center receives HHS funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
